How much time do you spend with each employee for the Online Assessment and Biometric Lab Screening? The Online Assessment takes approximately 15 minutes to complete and the screening takes approximately 10 minutes. Can you import HRA data from another vendor? We can import the hard data (Biometric Screening Data) assuming that it is in Excel or CSV format. We do not typically import data related to the self-reported information in the Health Risk Assessments. This is due to that fact that every self-reported HRA survey/questionnaire out there is slightly different in the questions asked and the algorithm it uses to process that data. In short, the data does not typically integrate properly and it compromises the comparison of the data. The best way to compare your results from a previous HRA is to look at the reports that were generated for you from that HRA and the reports that Tri Wellness will generate for you based on this year’s HRA. If it is imperative that self-reported data is imported, Tri Wellness can discuss the possibility of doing so and the costs involved. Are you licensed to provide Biometric Lab Screenings and what compliance documentation can you provide? The lab (and its examiners) that we use is a federally licensed facility and holds all licenses and certifications necessary to provide testing throughout the U.S. (all 50 states of the U.S.A.). How is your subcontractors/staff qualified? What credentials do they have, how long and how often do they conduct onsite Biometric Lab Screenings? Staffing for the biometric screenings is provided by companies within our lab’s national network. This network was created using occupational health clinics and hospital groups through their Toxicology and Wellness clients and paramedical firms through Insurance testing. It is requested by contractual obligation that all companies providing this service that those employees staffing our events:
For paramedical firms, training is conducted at the branch office level, using their specific training tool, policies and procedures. Training tools can include Examiner Training Manuals, Exam Best Practices Manuals, Paramed Handbook, OSHA Bloodborne Pathogen Exposure Control Plan and/or website training. The training includes how medical services are performed specifically for the insurance/wellness industry (venipuncture blood draws, physical measurements and OSHA guidelines for collecting, processing and handling specimens). Our lab also requires that each examiner go through in-service training specific to each client protocols prior to a screening event. Collection facilities and/or examiners must provide liability insurance. Our lab also maintains both general and professional liability in the amounts of $1 million per occurrence and $2 million aggregate. What can you tell us about your HIPAA compliance and practices? At Tri Wellness we recognize our obligation under the law, and our ethical obligations as a corporate citizen, to safeguard personal information provided to or generated by us. As a result, it is the Policy of Tri Wellness to collect, use and store non-public personal information (“NPI”) in a responsible manner. NPI shall mean any non-public personal and/or health information (whether written, oral, or electronic) of an individual. NPI shall be understood to include any personally identifiable health information created or received by us (including but not limited to, laboratory tests results, drug testing results, height, weight, blood pressure, pulse rate, prescription/medications the individual may have listed on a consent/chain of custody form, answers to health questions on consent/chain of custody form or health awareness questionnaire, etc.), and any personally identifiable financial information created or received by us (including but not limited to, individuals’ names and addresses, email addresses and phone numbers that are not publicly available, social security numbers, driver’s license number, credit card account numbers, bank account or other financial account numbers, and date of birth.) Tri Wellness will work to protect the confidentiality of NPI by complying with applicable State and Federal laws and regulations, and by taking reasonable steps to protect such data from unauthorized use or access. Tri Wellness maintains policies and procedures that describe the kind of information we collect, limit the use of that data to well-defined business purposes, and will not sell, disclose or use that data except for the business purposes for which it was gathered. Tri Wellness will ensure that persons who access that data will have a valid business purpose for that access, that persons use and disclose only the minimum necessary for all purposes other than for treatment purposes, and that all Tri Wellness employees, consultants, contractors and business associates agree to treat personal information according to the terms of a signed confidentiality agreement. Uses of personal data outside the limitations described above will be made only with the express consent of the individual(s) involved, or in compliance with applicable laws, or if all personally identifiable elements have been removed from the data. Uses or disclosure of NPI in a manner inconsistent with this policy will result in disciplinary action that may include termination. How do you ensure data accuracy and integrity for the biometric measurements? We require our collection staff to follow stringent protocol for collecting biometric measurements. All staff are trained using the lab’s collection protocol to ensure consistency and accuracy of biometric measurements. Once the forms and samples have been returned to the lab, the biometrics are entered by the date entry team. Each measurement requires a double blind entry to help ensure accuracy. In addition, error flags are automatically assigned to entries that fall outside of the expected parameters. For example, if someone’s height was entered as 8’0”, a flag would be created to hold the sample for review since the parameter cut off is 7’6”. If the person is indeed 8 feet tall, the flag would be removed and the sample released for reporting. How do you ensure that our employees have a great experience? With regards to the Online HRA, our portal was designed with user-friendliness in mind; the process is quite intuitive (no learning curve), the questions are straight-forward, it only takes 15 minutes to complete, and each participant will receive a simple, easy-to-read report upon completion. The HRA’s can be started, paused and restarted in case of interruption (very convenient). And in this case, the Biometric Lab Screenings and Online HRA’s will be conducted during a “same day experience” so participants can get their report quickly and so that the overall experience will be efficient.
