The TriWellness Connect HRA is a scalable, data-driven health risk assessment which uses evidence-based guidelines and has received Health Information Products Certification from the National Committee for Quality Assurance (NCQA) and complies with the Genetic Information Nondiscrimination Act (GINA).

Controlled Availablity

Clients can control the availability of the HRA by defining wellness cycles that determine when the HRA can be completed and when completion awards incentive points.

Integrated Incentives

Each organization can define the incentive points available for completion of the HRA.

Delivery Options

The HRA can be completed by the participant online or on paper using scannable bubble forms. Scanned data from paper forms is loaded into the platform database and merged with data collected through the online questionnaire, providing a single database for reporting.

Wellness coaches may complete the questionnaire online on behalf of the participant to support facilitated telephone delivery.

Language Support

The HRA questionnaire and participant reports are available in English and Spanish. Both versions use the same data elements, providing a single database for aggregate reporting and review by health coach.

Long form and Short Form

Tri Wellness connect HRA is available in a long form and short form:

  • The long form assesses current health, including lifestyle behaviors, biometrics, health status, and compliance with recommended preventive health screenings; chronic conditions, and future disease risk.

  • The short form of delivers all the functionality required to assess current health, including biometrics, but omits chronic and future disease risk assessments. The shorter format facilitates completion by populations with low literacy and reduces processing costs of paper assessments.

Organizations can deliver both long and short forms to the same population or subgroup and can switch between long and short forms from year to year. Aggregate reports support both versions when deployed within the same population and time period.

Assessments

Participants completing the TriWellness connect HRA are assessed for:

  • Lifestyle factors and readiness to change

  • Biometric and laboratory results

  • Compliance with recommended preventative screenings

  • Existing chronic conditions

  • Future disease risk

Biometric Data

Three options are available for entry of laboratory results and vital signs:

  • Self-reported by participant

  • Entry by health coaches

  • Batch load from file

Stratification

On completion of the HRA, participants can be stratified automatically into wellness and disease management programs using client-defined stratification rules. Stratification may include assignment of the participant to a coaching group and a personal coach for management using Coach Connect and enrollment in a Health Action Plan.
