Coach Connect (Included when Wellness Coaching is offered)

• This secure portal gives the employee several different ways to interact with their wellness coach.  Employees can view their coach’s availability, schedule appointments, send secure messages, and review progress notes.

• Coach Connect also allows the wellness coach to view Health Risk Assessment data and direct Action Plans for the employee.

Coach Connect also includes additional Self-Care Pages that the wellness coach can send directly to the participant via the Wellness Connect Messaging system.  Here is more information about the Self-Care Pages:

• Help participants make healthy lifestyle changes and avoid complications. The pages describe a condition, how it is treated, and what the appropriate self-care measures are. They explain medicine compliance and offer tips to help people improve their health

• One-page, two-sided, full-color educational materials—so they are easy to print and deliver online

• More than 400 topics are covered, including chronic conditions, prevention, and wellness

• Available in both English and Spanish

• Provides illustrated information that helps people do as much for themselves as they can; reinforcing what people have learned from their wellness coaches and doctors

• Integrated into the Wellness Connect portal to allow wellness coaches to share specific content related to their encounter, including medical tests, procedures, or medicines
